4-H Program Enrollment & Forms

Joining a Club or Becoming an Independent Member

Clover 101 is an introductory class for families interested in joining 4-H. Current 4-H’ers and volunteers will share about 4-H history, pledge and current program. Youth will explore project topics and options for exploring those. Families will get contact information about local clubs that will fit their needs. 4-H swag and idea books given to each family!

Youth can be enrolled in camps and classes in three ways:

Register and Pay with Credit Card online.

Register and Pay with Credit Card by phone at 704-920-3310

Register and pay with card, cash or check at the N.C. Cooperative Extension Office – 715 Cabarrus Avenue West in Concord.

All youth will  need to complete both the Enrollment Form and Medical Form to participate in class. The Medical Form needs to be Notarized. We do have a notary on staff at the 4-H Office, but please call to confirm availability. 704-920-3310
